Bone reconstruction and grafting

When there is not enough bone to support implants, we rebuild what is needed with advanced regenerative techniques.

Custom grid for bone regeneration
In short

When there is not enough bone to place an implant, it can be rebuilt with a bone graft or with guided bone regeneration techniques. In many cases of atrophy, however, grafting can be avoided or reduced thanks to subperiosteal implants with a custom grid (EagleGrid) or zygomatic implants. The quantity and quality of the bone are assessed in 3D with a cone beam CT scan before deciding.

Bone regeneration and grafting

When the bone is insufficient to hold an implant, we augment it with grafts and guided bone regeneration techniques. This recreates the base needed to support the implant in a stable, lasting way. We assess the quality and quantity of the bone with a cone beam CT scan before planning the procedure.

Why there may not be enough bone

Bone can shrink because of tooth loss that happened some time ago (bone that is no longer "stimulated" tends to resorb), periodontitis, infections or individual anatomy. Understanding the cause helps choose the most suitable reconstruction technique.

When a graft is needed

A graft is indicated when the height or thickness of the bone is not enough to place the implant safely. It is not always needed: we decide only after the 3D assessment, choosing the least invasive option that ensures a good result.

Maxillary sinus lift

In the back areas of the upper arch, when the bone height is insufficient, a sinus lift is often used: a technique that creates new bone where the implant will go. It can be performed through a lateral approach or, in simpler cases, through a crestal approach, at the same time as the implant is placed.

Alternatives to grafting

In many cases of atrophy we avoid or reduce grafting thanks to subperiosteal implants with a custom grid (EagleGrid), which rest on the existing bone, or zygomatic implants, which are anchored to the cheekbone (zygomatic bone). They also make it possible to restore the arch in a shorter time.

How it works and healing times

Depending on the case, the graft can be done at the same time as the implant placement or before it. When it comes first, a healing period is needed — generally a few months — for the new bone to mature. The exact timing depends on the extent of the reconstruction.

Risks and maintenance

Like any surgical procedure, bone regeneration carries possible risks, which we reduce through planning and explain to you while we design your treatment. Once the implant is in place, daily hygiene and regular checkups are essential to keep the result stable.

Frequently asked questions

Does a bone graft hurt?

The procedure is performed under local anesthesia, so no pain is felt during it. In the following days there may be mild swelling and discomfort, which can be managed with the prescribed medication.

Where does the bone used come from?

Depending on the case, graft materials of different origins are used or, sometimes, the patient's own bone. We explain the chosen solution and the reasons for it during planning.

How long before the implant can be placed?

It depends on the case: sometimes the graft is done at the same time as the implant, other times it comes first and requires a few months of healing for the new bone to mature.

Can a graft always be avoided?

Not always, but in many cases of atrophy alternatives such as EagleGrid and zygomatic implants make it possible to avoid or reduce it. We assess this with a cone beam CT scan.

Is it a safe procedure?

It is a well-established procedure; like any surgery it carries possible risks, which we reduce with careful 3D planning and explain to you before going ahead.
